Background: Neoadjuvant chemoradiation is the standard of care for locally advanced rectal cancer, however, pathological complete response (pCR) rate remains unsatisfactory. Tumor hypoxia in rectal cancer might be one factor of radioresistance. Mitomycin C (MMC), a hypoxia-activated prodrug (HAP), enhances radiosensitivity and is the standard adjunct to radiotherapy (RT) in anal cancer. Our study evaluated the effect of MMC and RT on rectal cancer and its tumor microenvironment (TME). Methods: In vitro hypoxia was induced by 1% O2 culture for CT26 rectal adenocarcinoma cells and HIF-1α expression was validated with Western blotting. Clonogenicity and MTT assays were used for radiation and drug survival. In syngeneic CT26 model, tumors were treated as follows: control, MMC (2 mg/kg/day), RT (2 Gy × 2 fractions), and combination with MMC 2 h prior to RT. Tumor volume, body weight, and white blood cell count were monitored for 1 month. Immune cells in TME were analyzed using flow cytometry. Anemia is prevalent among pregnant adolescents, but few data exist on biochemical indicators of iron status in this group. We hypothesized that among an at-risk population of African-American, pregnant adolescents, the degree of iron depletion and deficiency would be marked, and that iron deficiency anemia would comprise the majority of the observed anemia. To examine this, blood samples were collected from 80 girls (≤18 y old) attending an inner city maternity clinic, 23 of whom were studied longitudinally in the 2nd and 3rd trimesters depending on contact at the clinic. Sample sizes for the biomarkers varied according to the blood volume available at the time the assays were completed. Descriptive statistics were applied to characterize iron status, and multivariate regression and logistic analyses were used to identify significant determinants of iron status. Depleted iron stores (ferritin ≤ 15 μg/L) were indicated for 25% (n = 44) and 61% (n = 59) of adolescents during the 2nd and 3rd trimesters, respectively. Serum folate (39.3 ± 15.4 nmol/L, n = 60), RBC folate (2378 ± 971 nmol/L, n = 60), and serum vitamin B-12 concentrations (313 ± 163 pmol/L, n = 60) were within normal ranges. Adolescents with serum transferrin receptor:serum ferritin ratios (R:F ratio) > 300 during the 2nd trimester were 12.5 times (95% CI 2.83, 55.25) more likely to be classified with iron deficiency anemia during the 3rd trimester (P = 0.0002) than those with lower ratios. The present study shows that the sleep quality is disturbed during pregnancy and it is more in the third trimester of pregnancy, and pregnant women should be aware of the effects of sleep deprivation and adverse outcomes related to it. Background: Sleep disturbances are common during pregnancy. The risk of sleep disturbances is estimated to be two times higher in the later part of pregnancy compared to the early trimester. Thus, the present study was planned to assess the sleep quality in different trimesters of South Indian pregnant women.Methods: This was a descriptive cross-sectional questionnaire-based study. A total of 90 healthy pregnant women with a mean age of 25 years were recruited and sleep quality was assessed using a standardized PSQI questionnaire. Mean global PSQI score of ≥5 is suggestive of poor sleep quality. A p value <0.05 was considered statistically significant.Results: Subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleeping medications and daytime medication were found to be significantly higher in the second and third trimester (p<0.0001) pregnant women. Shengmai San (SMS, transliterally meaning “the decoction for restoring the pulse” in Chinese) is an herbal formulation for the treatment of conditions of body Qi-Yin deficiency, such as the depletion of Qi and body fluids under conditions of heat-stroke. SMS is comprised of Ginseng Radix (GR, a Qi-invigorating herb), Ophiopogonis Radix (OR, a Yin-nourishing herb) and Schisandrae Fructus (SF, an “astringent” herb). Recent pharmacological studies have revealed the biochemical basis of the Qi-invigorating and Yin-nourishing actions of the component herbs of SMS in relation to the pathological condition of heat-stroke. The pathogenesis of heat-stroke indicates that its major complications include brain ischemic injury, myocardial and respiratory dysfunction. In this regard, GR possesses a Qi-invigorating action which is related to the induction of a glutathione-dependent antioxidant response and the preservation of mitochondrial function, with the resultant protection against ischemia/reperfusion injury in the brain and heart during heat-stroke. OR is a Yin-nourishing herb, which produces immunomodulatory effects. OR can reduce the extent of pulmonary hypertension, edema and inflammation under heat-stress conditions. Cecal ulcers are sometimes encountered in asymptomatic individuals. Their clinical outcomes and management recommendations remain uncertain. Asymptomatic patients who underwent a colonoscopic exam for colon cancer screening were retrospectively reviewed from July 2009 to November 2016. Patients with cecal ulcers were included. Patients who had colorectal symptoms, such as abdominal pain, had nonsteroidal anti-inflammatory drugs or were lost to follow-up were excluded. A total of 34,036 patients underwent colon cancer screening. Cecal ulcers were found in 35 patients. After exclusion, 24 patients (mean duration, 52 months) received follow-up colonoscopy. In 20 patients, (83.3%), cecal ulcer resolved without intervention, but 4 patients (16.7%) developed clinical significant diseases, including intestinal tuberculosis (n = 2), Crohn’s disease (n = 1), and ulcerative colitis (n = 1). Patients who developed clinically significant diseases had a higher percentage of ulcers larger than 1 cm (75% vs. 15%, p = 0.035), terminal ileum involvement (100% vs. 15.4%, p = 0.006) and ulcers with irregular fold (75% vs. 5%, p = 0.008). In patients with asymptomatic cecal ulcers, the endoscopic features included larger ulcer size, terminal ileum involvement and ulcers with irregular fold may predict development of clinically significant diseases. If the above-mentioned features are present, even asymptomatic patients should be closely monitored.
